First appearance: World 6-1

Poison Mushroom is an item that made its first appearance on Super Mario Bros.: The Lost Levels (Super Mario Bros. 2 in Japan.) It's a power-down that shrinks a powered-up Mario to Small Mario and kills him when he's at his small form, eventually making it a trap for first-time players.

In Mario Forever, it makes its first appearance in World 6-1. Unlike The Lost Levels, touching it will instantly kill Mario, regardless of what power-up he has.

Appearance

A Poison Mushroom has a blue-skull on its purple cap, with angry eyes on its stem.

Effect

Once Mario collects the mushroom, he will immediately get killed.

A Poison Mushroom has the opposite effect of a Super Mushroom; it doesn't power-up Mario but rather kill him instantly, even with any power-ups (except for the Starman).

It makes its first appearance on World 6-1, and it's located before the level's Checkpoint. Upon jumping on the fourth ''?'' Block, the power-down will appear and start moving like a Super Mushroom.

It returns in Human-Laboratory-2 and The Lost Map-1. in Human Laboratory-2, there's two of them, and they, alongside other power-ups are secured with Big Clone Human Bs. While in The Lost Map-1, it's located in an invisible block with the Springboard.

In World of Stupidity-2 and World of Stupidity-3, the Poison Mushroom makes a return and comes with numbers, and they're usually located on ''?'' Blocks.
